ABSTRACT

Resumo O maior ramo da divisão terminal da artéria braquial é a artéria ulnar, que se origina após a fossa cubital. Essa artéria usualmente tem trajeto profundo aos músculos do antebraço anterior e é responsável pela vascularização da musculatura superficial e profunda da região ulnar do antebraço e hipotênar da mão, sendo a principal responsável pela formação do arco palmar superficial após o retináculo dos flexores. Reportamos uma variação anatômica após diagnóstico com ultrassom vascular na qual a artéria ulnar situava-se em posição superficial no antebraço. A ocorrência da artéria ulnar superficial é rara, porém de grande importância para clínicos, cirurgiões e profissionais de enfermagem.


Abstract The largest branch of the terminal division of the brachial artery is the ulnar artery, which arises after the cubital fossa. This artery usually has a deep path in the muscles of the anterior forearm and is responsible for vascularization of the superficial and deep musculature on the ulnar side of the forearm and hypothenar area of the hand. We report an anatomical variant diagnosed by Doppler ultrasound in which the ulnar artery had a superficial position in the forearm. Occurrence of a superficial ulnar artery is rare, but it is an important fact for clinicians, surgeons, and nursing professionals.

ABSTRACT

Resumo Contexto Pacientes com isquemia crítica (IC) dos membros inferiores (MMII) precisam de arteriografia para o planejamento da cirurgia de revascularização. A ultrassonografia Doppler (UD) não é invasiva e, através da aferição do índice de resistência (IR), pode fornecer informações sobre as artérias distais. Objetivos Correlacionar a Classificação Angiográfica de Rutherford com o IR na avaliação do leito arterial distal dos MMII. Métodos Estudo transversal, realizado em hospital público terciário, com 120 pacientes portadores de IC dos MMII, entre setembro de 2019 a abril de 2022. Foi comparado o IR das artérias da perna passíveis de serem receptoras de revascularização com a imagem obtida através da arteriografia dessas artérias em acordo com a Classificação Angiográfica de leito distal de Rutherford. Resultados Foram avaliados 120 MMII em 120 pacientes com idade média de 68,6 anos. A amostra foi composta de 50,0% de pacientes do sexo masculino. Na amostra, 90,0% pacientes encontravam-se na classe cinco de Rutherford. Os valores do IR encontrados para as artérias de perna apresentaram uma correlação positiva, estatisticamente significativa, quando comparados com a Classificação de Rutherford (tibial anterior, p< 0,01; tibial posterior, p = 0,012 e fibular, p = 0,034 e artéria dorsal do pé, p < 0,001). Conclusões Neste estudo, os IRs das artérias da perna obtidos através da ultrassonografia Doppler apresentaram uma correlação positiva quando comparados à classificação de Rutherford. Em pacientes com isquemia crítica, esse índice pode ser útil na avaliação do leito arterial distal dos membros inferiores.


Abstract Background Patients with chronic limb threatening ischemia (CLTI) of the lower limbs (LL) undergo arteriography for revascularization surgery planning. Doppler ultrasound (DU) is non-invasive and can provide information about the distal arteries through measurement of the resistance index (RI). Objectives To correlate the Rutherford Angiographic Classification with the RI for assessment of the distal arterial bed of the LL. Methods A cross-sectional study, conducted at a public tertiary hospital with 120 patients with LL CLTI, from September 2019 to April 2022. The RI of arteries that were candidates for revascularization was compared with the images of the same arteries obtained using arteriography, using the Rutherford Angiographic Classification of the distal bed. Results A total of 120 LL were assessed in 120 patients with a mean age of 68.6 years. The sample was 50.0% male and 90.0% of the patients in the sample were classified as Rutherford category five. The RI values found for the arteries of the leg exhibited a statistically significant positive correlation with the Rutherford Classification (anterior tibial, p< 0.01; posterior tibial, p = 0.012 fibular, p = 0.034; and dorsalis pedis, p < 0.001). Conclusions In this study, RIs for the arteries of the leg measured using Doppler ultrasound exhibited a positive correlation with the Rutherford Classification. This index could be useful for assessment of the distal arterial bed of the lower limbs of patients with chronic limb threatening ischemia.

ABSTRACT

Objetivo: Determinar los hallazgos por ecografía Doppler arterial oftálmica en pacientes con enfermedad renal crónica avanzada. Métodos: Se efectuó un estudio observacional descriptivo y transversal con 212 órbitas de 106 pacientes con enfermedad renal crónica avanzada (estadios 4 y 5 en tratamiento dialítico). Por interrogatorio y examen físico se identificaron los factores de riesgo aterosclerótico. Además, se realizó ultrasonido orbitario y Doppler carotídeo, y solo en caso de resultar normales, se procedió a evaluar mediante ecografía Doppler las arterias oftálmicas. Resultados: Predominaron los pacientes mayores de 50 años, el sexo masculino, el color mestizo de piel y los normopesos; mientras que la hipertensión arterial, el tabaquismo y la diabetes mellitus tipo 2 fueron los factores de riesgo aterosclerótico mayoritarios. En todos los enfermos renales crónicos se demostró un incremento de la velocidad del flujo y de la resistencia vascular a nivel de las arterias oftálmicas, en tanto los casos con hipertensión arterial y diabetes mellitus tipo 2, así como los hipertensos exclusivos, mostraron los valores hemodinámicos más elevados. Conclusiones: La evaluación de las arterias oftálmicas mediante ecografía Doppler permite hacer un estudio y seguimiento más integral de los pacientes con enfermedad renal crónica avanzada.


Objective: To determine ophthalmic arterial Doppler ultrasound findings in patients with advanced chronic kidney disease. Methods: A descriptive and cross-sectional observational study was carried out with 212 orbits of 106 patients with advanced chronic kidney disease (stages 4 and 5 in dialysis treatment). Atherosclerotic risk factors were identified by interrogation and physical examination. In addition, orbital ultrasound and carotid Doppler were performed, and only if they were normal, the ophthalmic arteries were evaluated by Doppler ultrasound. Results: Patients older than 50 years, male sex, mestizo skin color and normal weight predominated, while arterial hypertension, smoking and type 2 diabetes mellitus were the main atherosclerotic risk factors. An increase in flow velocity and vascular resistance at the level of the ophthalmic arteries was demonstrated in all chronic renal patients, while cases with arterial hypertension and type 2 diabetes mellitus, as well as exclusive hypertensives, showed the highest hemodynamic values. Conclusions: The evaluation of the ophthalmic arteries by Doppler ultrasound allows a more comprehensive study and follow-up of patients with advanced chronic kidney disease.

ABSTRACT

El Síndrome de Klippel-Trenaunay se constituye en una malformación vascular compleja con una incidencia de 2 a 3 casos por cada 100.000 nacidos vivos, clínicamente presenta una triada clásica: manchas cutáneas en vino de Oporto, venas varicosas de localización atípica e hipertrofia ósea y de tejidos blandos. Se presenta el caso de una paciente femenina de 33 años, sin antecedentes patológicos o quirúrgicos de importancia quien acude a consulta por aumento de volumen de la extremidad derecha, mancha violácea ipsilateral, además de dolor y parestesias. Al examen físico se evidencia aumento longitudinal de miembro inferior derecho, nevus hiperpigmentario en cara lateral de pierna derecha y muslo que se extiende al glúteo ipsilateral además de venas varicosas atípicas. Se practica ecografía Doppler venosa con transductor lineal que reporta incompetencia de vena Safena Mayor y perforantes suprageniculares. Se realizó manejo quirúrgico mediante safenectomía, corrección de deformidad y referencia a dermatología para terapia láser por el nevus hiperpigmentario. La paciente mostró evolución clínico - quirúrgica favorable con remisión de la sintomatología que motivó su consulta.


Klippel-Trenaunay syndrome is a complex vascular malformation with an incidence of 2 to 3 cases per 100,000 live births. Clinically, it presents a classic triad: port-wine stains, varicose veins of atypical location and bone and soft tissue hypertrophy. We present the case of a 33-year-old female patient, with no pathologic or surgical history of importance, who comes to the clinic for an increase in volume of the right extremity, ipsilateral violaceous spot, in addition to pain and paresthesia. Physical examination revealed longitudinal enlargement of the right lower limb, hyperpigmented nevus on the lateral aspect of the right leg and thigh extending to the ipsilateral buttock and atypical varicose veins. Venous Doppler ultrasound with linear transducer reported incompetence of the greater saphenous vein and supragenicular perforators. Surgical management was performed by saphenectomy, deformity correction and referral to dermatology for laser therapy for hyperpigmented nevus. The patient showed favorable clinical-surgical evolution with remission of the symptomatology that motivated her consultation.


A síndrome de Klippel-Trenaunay é uma malformação vascular complexa com uma incidência de 2 a 3 casos por 100.000 nascidos vivos. Clinicamente, apresenta uma tríade clássica: manchas vinho do porto, veias varicosas de localização atípica e hipertrofia óssea e de tecidos moles. Apresentamos o caso de uma paciente do sexo feminino, 33 anos, sem histórico patológico ou cirúrgico de importância, que consultou por aumento de volume do membro direito, mancha violácea ipsilateral, além de dor e parestesia. O exame físico revelou aumento longitudinal do membro inferior direito, nevo hiperpigmentado na face lateral da perna e coxa direitas, estendendo-se até a nádega ipsilateral e veias varicosas atípicas. Foi realizado ultrassom Doppler venoso com transdutor linear, que relatou incompetência da veia safena magna e das perfurantes suprageniculares. O tratamento cirúrgico foi realizado por meio de safenectomia, correção da deformidade e encaminhamento à dermatologia para terapia a laser para nevo hiperpigmentado. A paciente apresentou uma evolução clínico-cirúrgica favorável com remissão dos sintomas que a levaram à consulta.

ABSTRACT

Purpose: To examine the role of the strain ratio in elastosonography for the differential diagnosis of common intraocular tumors such as choroidal melanoma, choroidal hemangioma, choroidal metastatic carcinoma, and retinoblastoma. Methods: This study included patients suffering from intraocular space?occupying lesions and who visited Beijing Tongren Eye Center of Beijing Tongren Hospital affiliated to Capital Medical University from June 2016 to March 2020. All patients underwent a physical examination, fundus examination with mydriasis, color Doppler ultrasonography, elastosonography, magnetic resonance imaging (MRI), and fundus angiography within 1 week. All patients were grouped as choroidal melanoma, choroidal metastatic carcinoma, retinoblastoma, choroidal hemangioma, and optic disk melanocytoma. A receiver operating characteristic (ROC) curve analysis was performed to assess the strain ratio for diagnosing malignant intraocular tumors. Results: A total of 155 patients (161 eyes) were recruited. The strain ratios measured were 39.59 ± 15.92 for choroidal melanoma, 36.85 ± 13.64 for choroidal metastatic carcinoma, 38.93 ± 17.27 for retinoblastoma, 13.42 ± 10.93 for choroidal hemangioma, and 3.84 ± 1.32 for optic disk melanocytoma. The strain ratios of the three malignant lesions were significantly higher than those of the two benign lesions (all P < 0.001). The area under the ROC curve was 0.95 ± 0.028. The optimal cutoff point was 22.67, with 85.7% sensitivity and 96.4% specificity. Conclusion: There were significant differences in elasticity between the malignant and benign intraocular tumors. The strain ratio using elastosonography could serve as an important auxiliary examination to distinguish between benign and malignant intraocular tumors

Background@#Gestational trophoblastic neoplasia (GTN) presents as vascular mass of varying morphology on ultrasound and confirmed through quantitative serum β subunit human chorionic gonadotrophin (β‑hCG). In regions with limited access to β‑hCG, ultrasound plays a crucial role in the initial diagnosis for timely management.@*Objectives@#This study aimed to investigate the associations between ultrasound vascular morphologic features, serum β‑hCG levels, and histopathology in GTN cases.@*Methodology@#A cross‑sectional review was conducted on 113 cases with ultrasound impression of GTN over an 8‑year period. The patient data were extracted from case records, and ultrasound images were categorized based on the distinct features. Associations with β‑hCG levels and histopathology were analyzed using the Chi‑square test and Mann–Whitney U‑test. Statistical significance was set at P < 0.05.@*Results@#A significant association was observed between ultrasound category and serum β‑hCG (P < 0.0001). The compact and diffuse types were more prevalent with β‑hCG levels between 104 to <105 mIU/mL, while the lacunar type was common among patients with ≥105 mIU/mL. However, there was no significant association between ultrasound categories and mean β‑hCG levels. Regarding histopathology, the lacunar type was more common in invasive moles, whereas the compact and diffuse types tended to be seen with choriocarcinoma, although these were not statistically significant (P = 0.182).@*Conclusions@#Morphologic types of GTN by gray scale and Doppler ultrasound vary across the different levels of serum β‑hCG and may suggest the histopathological diagnosis. This study provides valuable insights into the ultrasonographic characteristics of GTN, which can aid in its diagnosis and management.

ABSTRACT

Resumo Fatores relativos à placa aterosclerótica podem indicar instabilidade como ulcerações, hemorragias intraplaca, núcleo lipídico, capa fibrosa delgada ou irregular e inflamação. A mediana de escala de cinza (GSM, de greyscale median) da placa é um dos métodos mais difundidos de estudo da placa aterosclerótica; nesse sentido, é importante criar uma padronização da pós-processamento de forma compreensível. O pós-processamento foi realizado no software Photoshop 23.1.1. A padronização da imagem foi alcançada com o ajuste de curvas do histograma de escalas de cinza definindo o ponto mais escuro do lúmen vascular (sangue) para zero e a adventícia distal para 190. A posterização e o remapeamento de cores foram realizados. Um método que apresenta o atual estado da arte da técnica de forma acessível e ilustrativa pode contribuir para disseminação da análise de GSM. Neste artigo, esse processo é demonstrado passo a passo.


Abstract Factors related to atherosclerotic plaques may indicate instability, such as ulcerations, intraplaque hemorrhages, lipid core, thin or irregular fibrous cap, and inflammation. The grayscale median (GSM) value is one of the most widespread methods of studying atherosclerotic plaques and it is therefore important to comprehensively standardize image post-processing. Post-processing was performed using Photoshop 23.1.1.202. Images were standardized by adjusting the grayscale histogram curves, setting the darkest point of the vascular lumen (blood) to zero and the distal adventitia to 190. Posterization and color mapping were performed. A methodology that presents the current state of the art in an accessible and illustrative way should contribute to the dissemination of GSM analysis. This article describes and illustrates the process step by step.

ABSTRACT

Introducción. La variación de la velocidad máxima aórtica con la ventilación mecánica (ΔVpeakAo) ha demostrado ser el mejor predictor de respuesta a volumen en pediatría. Existe evidencia en adultos de que la variación de velocidad máxima de flujo carotídeo (ΔVpeakCar) es predictor de respuesta a fluidos. Al momento es escasa la información sobre este índice en pediatría. Su beneficio se basa en la no-invasividad, y que para su medición no es necesaria la ecocardiografía ni el acceso al tórax del paciente. Objetivo. El objetivo general de este trabajo fue estudiar la correlación y la concordancia de ΔVpeakCar con ΔVpeakAo en una población pediátrica bajo ventilación mecánica. Metodología. Se incluyeron pacientes de 0 a 12 años. Se registraron flujos aórtico y carotídeos máximos y mínimos y se calculó ΔVpeakCar y ΔVpeakAo. Para analizar correlación y concordancia entre las variables se utilizó el test de Pearson, análisis de Bland-Altman y análisis de los 4-cuadrantes. Resultados. Se estudiaron 58 pacientes, 13 lactantes (menores 12 meses), 21 preescolares (12-60 meses) y 24 escolares (mayores a 60 meses). Se observó una correlación significativa entre ΔVpeakAo y ΔVpeakCar (r=0,85; p<0,05) con un coeficiente de determinación de r2=0,72. El análisis de Bland-Altman mostró un sesgo del 0,15% (IC95%, -0.7-1.0) con un límite de concordancia del -6,1 a 6,2%. La concordancia fue 85%, con un sesgo angular de 4,5°±31°. El análisis por subgrupos mostró un r2 de 0.89 en escolares, 0.56 en preescolares y 0.45 en lactantes. La concordancia fue de 100% en escolares, 95% en prescolares y 93% en lactantes. Discusión y conclusiones. El registro de ΔVpeakCar fue viable. Al analizar la capacidad de ΔVpeakCar de sustituir a ΔVpeakAo en el total de la muestra, no es buena La correlación y concordancia son mejores en escolares. Es necesario continuar estudiando este nuevo índice.


Introduction. The variation in maximum aortic velocity with mechanical ventilation (ΔVpeakAo) has proven to be the most effective predictor of fluid response in pediatrics. While there is evidence in adults that the variation in maximum carotid flow velocity (ΔVpeakCar) predicts fluid response, information on this index in pediatrics remains limited. Its advantage lies in its non-invasive nature, eliminating the need for echocardiography or thoracic access for recording. Objective. This study aims to examine the correlation and concordance between ΔVpeakCar and ΔVpeakAo in a pediatric population. Methodology. The study included patients aged 0 to 12 years. Maximum and minimum aortic and carotid flows were recorded, and ΔVpeakCar and ΔVpeakAo were calculated. Correlation and agreement between variables were analyzed using the Pearson test, Bland Altman analysis, and 4-quadrant analysis. Results. A total of 58 patients were studied, comprising 13 infants (under 12 months), 21 preschoolers (12-60 months), and 24 school-aged children (over 60 months). A significant correlation was observed between ΔVpeakAo and ΔVpeakCar (r=0.85; p<0.05) with a coefficient of determination, r²=0.72. The Bland-Altman analysis revealed a bias of 0.15% (95% CI, -0.7-1.0) with an agreement limit of -6.1% to 6.2%. The concordance rate was 85%, with an angular bias of 4.5°±31°. Subgroup analysis showed r² values of 0.89 in school-aged children, 0.56 in preschoolers, and 0.45 in infants. Concordance rates were 100% in school-aged children, 95% in preschoolers, and 93% in infants. Discussion and Conclusions. The measurement of ΔVpeakCar proved feasible. However, when considering its ability to replace ΔVpeakAo, the results are suboptimal. Correlation and concordance are stronger in school-aged children. Further investigation into this new index is warranted.

ABSTRACT

Objective:To compare the clinical value of CTA and CDU in perforator flap of peroneal artery.Methods:From February 2013 to October 2016, 47 patients who suffered with soft tissue defects and were hospitalised in the Department of Orthopaedics, the 920th Hospital of Joint Logistic Support Force of Chinese PLA were retrospectively reviewed and evaluated. All the defects were reconstructed by the perforator flap of peroneal artery. All patients received preoperative CTA and CDU scans before surgery. Appropriate perforator vessels were selected and the locating points in body surface and external diameters of the perforator vessels were recorded and compared with intraoperative findings. SPSS 22.0 statistical software was used for data analysis. P<0.05 was considered statistically significant. Results:The intraoperative coincidence rate of the proposed perforator vessels was 97.87% for CDU and 95.74% for CTA, with no significant difference between the 2 groups( P>0.05). It was found that the preoperative CTA and CDU measurements were consistent with the actual intraoperative measurements, and there was no significant difference between the 2 groups( P>0.05). For CTA combined with CDU, an intraoperative coincidence rate was 100% in the location of peroneal perforating vessels. All flaps were followed-up for 1 to 18(mean 13.5) months. All the flaps survived well with good texture and appearance without complication. Conclusion:CDU and CTA are reliable and useful in preoperative vascular evaluation of peroneal perforator flap, and both can be used in a complementary or combined manner.

Introducción: La diabetes mellitus es la enfermedad crónica considerada como una de las mayores emergencias sanitarias del siglo XXI en el mundo, que puede desencadenar algunas complicaciones y problemas graves para la salud. Objetivo: Caracterizar a pacientes diabéticos con enfermedad arterial crónica en miembros inferiores según variables clínicas, hemodinámicas y ecográficas. Métodos: Se realizó un estudio observacional, descriptivo y transversal de los 178 pacientes diabéticos con diagnóstico clínico de enfermedad arterial crónica en miembros inferiores, quienes fueron atendidos en el Servicio de Imagenología del Hospital General Docente Dr. Juan Bruno Zayas Alfonso de Santiago de Cuba desde enero de 2017 hasta diciembre de 2019, a los cuales se les realizó examen hemodinámico mediante el índice tobillo-brazo y ecografía Doppler arterial. Resultados: En la investigación primaron los adultos mayores (41,6 %) del sexo femenino (54,0 %), la claudicación intermitente como principal síntoma (57,3 %), el sector femoropoplíteo (34,8 %) según la topografía exacta de las lesiones, así como la estenosis significativa (53,9 %) y la enfermedad arterial periférica moderada (58,3 %). Conclusiones: La mayoría de los pacientes presentó una estenosis significativa detectada por ecografía Doppler, en correspondencia con el diagnóstico de enfermedad arterial periférica moderada por el índice tobillo-brazo. La ecografía Doppler también aportó la topografía exacta de las lesiones.


Introduction: The diabetes mellitus is the chronic disease considered as one of the highest sanitary emergencies in the XXI century in the world that can trigger some complications and serious health problems. Objective: To characterize diabetic patients with arterial chronic disease in lower limbs according to clinical, hemodynamic and ultrasound variables. Methods: An observational, descriptive and cross-sectional study of the 178 diabetic patients with clinical diagnosis of arterial chronic disease in lower limbs was carried out, who were assisted in the Imaging Service of Dr. Juan Bruno Zayas Alfonso General Hospital in Santiago de Cuba from January, 2017 to December, 2019, to whom hemodynamic exam by means of the ankle-brachial index and arterial Doppler ultrasound was carried out. Results: In the investigation there was a prevalence of the elderly (41.6 %) from the female sex (54.0 %), the intermittent abandonment as main symptom (57.3 %), the femoropopliteal sector (34.8 %) according to the exact topography of the lesions, as well as the significant stenosis (53.9 %) and the moderate peripheral arterial disease (58.3 %). Conclusions: Most of the patients presented a significant stenosis according to Doppler ultrasound, in correspondence with the diagnosis of moderated peripheral arterial disease by the ankle-brachial index. The Doppler ultrasound also contributed the exact topography of the lesions.

Objetivo: Determinar los hallazgos de retinopatía diabética por ecografía doppler arterial oftálmico en pacientes con diabetes mellitus tipo 2. Métodos: Se efectuó un estudio observacional descriptivo y transversal en 200 órbitas de 100 pacientes diabéticos, a los que se les realizó fondo de ojo para diagnóstico y gradación de la retinopatía diabética. Por interrogatorio y examen físico se determinó el tiempo de diagnóstico de la diabetes y la presencia o ausencia de obesidad. Además, se les hizo ultrasonido orbitario y Doppler carotídeo, y solo en caso de resultar normales, se procedió a evaluar mediante ecografía doppler a las arterias oftálmicas. Resultados: Predominaron los diabéticos entre 40 y 60 años, el sexo masculino, los casos sin retinopatía diabética, y el grupo de más de 10 años de diagnóstico de la diabetes. Se demostró una disminución de la velocidad del flujo y un incremento de la resistencia vascular a nivel de las arterias oftálmicas, en tanto se evidenció asociación significativa de estos parámetros hemodinámicos con el envejecimiento, la presencia o ausencia de obesidad, el tiempo de diagnóstico de la diabetes y la severidad de la retinopatía diabética. Sin embargo, no se encontró asociación significativa con el sexo. Conclusiones: La valoración mediante ecografía doppler de las arterias oftálmicas, haría más completo el estudio y seguimiento de los pacientes con diabetes mellitus tipo 2(AU)


Purpose: To determine the findings of diabetic retinopathy by ophthalmic arterial Doppler ultrasound in patients with type 2 diabetes mellitus. Methods: A descriptive and cross-sectional observational study was carried out in 200 orbits of 100 diabetic patients, who underwent fundus examination for diagnosis and grading of diabetic retinopathy. The time of diagnosis of diabetes and the presence or absence of obesity were determined by interrogation and physical examination. In addition, orbital ultrasound and carotid Doppler were performed, and only if they were normal, the ophthalmic arteries were evaluated by Doppler ultrasound. Results: Diabetic patients between 40 and 60 years of age, from the male gender, cases without diabetic retinopathy, and the group with more than 10 years of diagnosed diabetes predominated. A decrease in flow velocity and an increase in vascular resistance at the level of the ophthalmic arteries were demonstrated, while a significant association of these hemodynamic parameters with aging, presence or absence of obesity, time of diagnosis of diabetes and severity of diabetic retinopathy was evidenced. However, no significant association was found as to gender. Conclusions: Doppler ultrasound assessment of the ophthalmic arteries would make the study and follow-up of patients with type 2 diabetes mellitus more complete(AU)

ABSTRACT

Introducción. El uso del Doppler transcraneal (DTC) en pacientes neurocríticos se reporta cada vez más en las terapias intensivas pediátricas. El objetivo de esta encuesta es conocer los usos y prácticas del DTC en la atención neurocrítica y el proceso de formación del personal que realiza el estudio. Materiales y métodos. Encuesta a profesionales de 23 unidades de terapia intensiva pediátrica de Argentina. Resultados. Porcentaje de respuesta del 86 %. Se utilizó en sospecha de muerte encefálica (n = 20), trauma de cráneo (n = 16) y accidente cerebrovascular (n = 16). El intensivista pediátrico es quien realiza los estudios (n = 13/20). Los encuestados utilizan el Doppler para decidir conductas y tratamientos, comenzar el proceso de evaluación de muerte encefálica, solicitar tomografía de cerebro y manejar la presión de perfusión cerebral con vasopresores. Conclusión. Todos los encuestados utilizan los hallazgos del DTC para guiar tratamientos o conductas. La mitad de los encuestados está poco conforme con la capacitación recibida.


Introduction. The use of transcranial Doppler (TCD) ultrasoundin neurocritical patients is reported to be increasingly common in pediatric intensive care units. The objective of this survey was to know about the use and practice of TCD ultrasound in neurocritical care and the training process of staff members performing it. Materials and methods. Survey administered to providers from 23 pediatric intensive care units of Argentina. Results. The percentage of response was 86%. TCD ultrasound was used for suspected brain death (n = 20), head injury (n = 16), and stroke (n = 16). Pediatric intensivists perform the test (n = 13/20). Surveyed participants use TCD ultrasound to decide on treatment and management, start brain death assessment, request brain computed tomography, and manage cerebral perfusion pressure with vasopressors. Conclusion. All surveyed participants use TCD ultrasound findings to guide management or treatments. Half of surveyed participants are little satisfied with their training.

Objetivo: Determinar los hallazgos por ecografía Doppler arterial oftálmica en pacientes con hipertensión arterial primaria. Métodos: Se efectuó un estudio observacional descriptivo y transversal con 202 órbitas de 101 pacientes hipertensos, a los que se les realizó fondo de ojo para diagnóstico y gradación de la retinopatía hipertensiva. Por interrogatorio y examen físico se identificaron como factores de riesgo aterosclerótico el consumo excesivo de alcohol y la obesidad. Además, se les hizo ultrasonido orbitario y Doppler carotídeo, y solo en caso de resultar normales, se procedió a evaluar mediante ecografía Doppler las arterias oftálmicas. Resultados: Predominaron los hipertensos entre 40 y 60 años de edad, el sexo masculino, el color mestizo de piel, los casos sin retinopatía hipertensiva, y el grupo entre 10 y 20 años de diagnosticada la hipertensión. Se demostró un incremento de la velocidad del flujo y de la resistencia vascular a nivel de las arterias oftálmicas. En tanto, se evidenció asociación significativa de estos parámetros hemodinámicos con el envejecimiento y el tiempo de diagnóstico de la hipertensión. Sin embargo, no se comprobó asociación significativa con el sexo, el color de la piel, y la presencia o ausencia de obesidad, consumo excesivo de alcohol, y retinopatía hipertensiva independientemente de su gravedad. Conclusiones: La evaluación de las arterias oftálmicas mediante ecografía Doppler permite hacer un estudio y seguimiento más integral de los pacientes con hipertensión arterial primaria(AU)

Methods: A descriptive and cross-sectional observational study was carried out with 202 orbits of 101 hypertensive patients, who underwent fundus examination for diagnosis and grading of hypertensive retinopathy. By interrogation and physical examination, excessive alcohol consumption and obesity were identified as atherosclerotic risk factors. In addition, orbital ultrasound and carotid Doppler were performed, and only if they were normal, the ophthalmic arteries were evaluated using Doppler ultrasound. Results: Hypertensive patients between 40 and 60 years of age, male, mixed skin color, cases without hypertensive retinopathy, and the group between 10 and 20 years after hypertension diagnosis predominated. An increase in flow velocity and vascular resistance at the level of the ophthalmic arteries was demonstrated. Meanwhile, a significant association of these hemodynamic parameters with aging and time of diagnosis of hypertension was evidenced. However, no significant association was found with gender, skin color, and the presence or absence of obesity, excessive alcohol consumption, and hypertensive retinopathy regardless of its severity. Conclusions: Evaluation of the ophthalmic arteries using Doppler ultrasound allows a more comprehensive study and follow-up of patients with primary arterial hypertension(AU)

ABSTRACT

Background: The main purpose of providing antenatal care is to identify women at risk ofcomplications as a result of impaired uteroplacental circulation such as pre-eclampsia.Aim: To describe practical application of Doppler ultrasound in obstetrics with analysis of bloodflow velocity waveform in pre-eclampsia and to examine perinatal outcome in pregnancies withaltered Doppler indices.Material and Method: This is a prospective study of 40 pregnant women with pre-eclampsiaevaluated sono-graphically with color Doppler and to be followed subsequently for any adverseperinatal outcome.Result: Out of 40 cases 45% of cases were found in 25-29 years age group. 25 cases had abnormaluterine artery Doppler indices accounting for 62.5%, while 15 cases had a normal doppler indicesaccounting for 37.5%. Out of 40 cases, 24 cases had abnormal Middle cerebral artery accounting for60%. In our study 14 cases had abnormal umbilical artery Doppler indices accounting for 35%,while 26 cases had a normal Doppler index accounting for 65%. In this study total 6 cases (15%) ofperinatal mortality were seen.Conclusion: The knowledge of Doppler arteries waveform may help to improve pregnancyoutcome and any permit identification and assessment of pre-eclampsia at earliest gestational age ascompared to other ante partum test modalities.

RESUMEN La ecografía Doppler transcraneal es un método no invasivo que permite una adecuada monitorización de los diferentes parámetros que ayudan a definir conductas para los médicos intensivistas, sin embargo, su utilización no está generalizada entre las comunidades médicas que atienden niños con afecciones neurocríticas. Es propósito de los autores, actualizar el tema en estudio y presentar su experiencia en población pediátrica. Las indicaciones de este método provienen de investigaciones en pacientes adultos, se necesitan estudios multicéntricos en diferentes contextos clínicos para poder establecer esta técnica como un método de diagnóstico confiable en pacientes pediátricos. Concluimos que utilizar el Doppler transcraneal como prueba auxiliar en la estimación de la presión intracraneal y presión de perfusión cerebral, proporciona adoptar recursos terapéuticos frente al paciente lo más acertados posibles y brinda la posibilidad de hacer un seguimiento y evaluación de los tratamientos a pie de cama de forma mínimamente invasiva.


ABSTRACT Transcranial Doppler ultrasound is a non-invasive method that allows adequate monitoring of the different parameters that help define behaviors for intensivist physicians ; however, its use is not widespread among the medical communities that care for children with neurocritical conditions. It is the purpose of the authors to update the topic under study and present their experience with pediatric populations. The indications for this method come from research in adult patients . Multicenter studies in different clinical contexts are needed to establish this technique as a reliable diagnostic method in pediatric patients. We conclude that using transcranial Doppler as an auxiliary test in the estimation of intracranial pressure and cerebral perfusion pressure, provides the implementation of therapeutic resources in front of the patient as accurate as possible and offers the possibility of monitoring and evaluating bedside treatments in a minimally invasive way.

ABSTRACT

Objetivos: determinar el desempeño predictivo de la definición de retardo de crecimiento fetal (RCF) de ultrasonografía de la Sociedad de Medicina Materno Fetal (SMMF), consenso Delphi (CD) y Medicina Fetal de Barcelona (MFB) respecto a resultados adversos perinatales en cada una, e identificar si hay asociación entre diagnóstico de RCF y resultados adversos perinatales. Materiales y métodos: se realizó un estudio de cohorte retrospectiva. Se incluyeron gestantes con embarazo único de 24 a 36 semanas con 6 días, quienes fueron atendidas en la unidad de medicina materna fetal con evaluación ecográfica de crecimiento fetal y atención de parto en una institución hospitalaria pública de referencia ubicada en Popayán, Colombia. Se excluyeron embarazos con hallazgos ecográficos de anomalías congénitas. Muestreo por conveniencia. Se midieron variables sociodemográficas y clínicas de las gestantes al ingreso, la edad gestacional, el diagnóstico de RCF y el resultado adverso perinatal compuesto. Se analizó la capacidad predictiva de tres criterios diagnósticos de restricción de crecimiento fetal para malos resultados perinatales y la asociación entre el diagnóstico de RCF y mal resultado periantal. Resultados: se incluyeron 228 gestantes, cuya edad media fue de 26,8 años, la prevalencia de RCF según los tres criterios fue de 3,95 %, 16,6 % y 21,9 % para CD, MFB y SMMF respectivamente. Ningún criterio aportó área bajo la curva aceptable para predicción de resultado neonatal adverso compuesto, el diagnóstico de RCF por CD y SMMF se asoció a resultados adversos perinatales con RR de 2,6 (IC 95 %: 1,5-4,3) y 1,57 (IC 95 %: 1,01-2,44), respectivamente. No se encontró asociación por MFB RR: 1,32 (IC 95 %: 0,8-2,1). Conclusiones: ante un resultado positivo para RCF, el método Delphi se asocia de manera más importante a los resultados perinatales adversos.Los tres métodos tienen una muy alta proporción de falsos negativos en la predicción de mal resultado perinatal. Se requieren estudios prospectivos que reduzcan los sesgos de medición y datos ausentes.


Objectives: To determine the predictive performance of fetal growth restriction by Maternal Fetal Medicine Society (MFMS) definition of ultrasound, the Delphi consensus (DC) and the Barcelona Fetal Medicine (BFM) criteria for adverse perinatal outcomes, and to identify whether there is an association between the diagnosis of fetal growth restriction (FGR) and adverse perinatal outcomes. Material and methods: A retrospective cohort study was conducted including women with singleton pregnancies between 24 and 36 weeks of gestation seen at the maternal fetal medicine unit for ultrasound assessment of fetal growth and delivery care in a public referral hospital in Popayán, Colombia. Pregnancies with ultrasound findings of congenital abnormalities were excluded. Convenience sampling was used. Sociodemographic and clinical variables were measured on admission; additional variables were gestational age, FGR diagnosis and adverse composite perinatal outcome. The predictive ability of three fetal growth restriction diagnostic criteria for poor perinatal outcomes was analyzed and asociation between FGR and adverse perinatlal outcomes. Results: Overall, 228 pregnant women with a mean age of 26.8 years were included; FGR prevalence according to the three criteria was 3.95 %, 16.6 % and 21.9 % for DC, BFM and MFMS, respectively. None of the criteria resulted in an acceptable area under the curve for the prediction of the composite adverse neonatal outcome; FGR diagnosis by DC and MFMS were associated with adverse perinatal outcomes with a RR of 2.6 (95 % CI: 1.5-4.3) and 1.57 (95 % CI: 1.01-2.44) respectively. No association was found for BFM RR: 1.32 (95 % CI: 0.8-2.1). Conclusions: Given a positive result for FGR, the Delphi method is significantly associated with adverse perinatal outcomes. The proportion of false negative results for a poor perinatal outcome is high for the three methods. Prospective studies that reduce measurement and attrition bias are required.

Abstract Introduction: Fine needle aspiration cytology is preferred for thyroid nodules preoperatively, but has disadvantages of false-negative and false-positive results. Objective: To compare the diagnostic performance of grayscale ultrasound, subjective color Doppler ultrasound, and combined features of grayscale ultrasound and subjective color Doppler ultrasound in predicting thyroid carcinoma, using results of the fine needle aspiration cytology as the reference standard. Methods: Data from gray-scale ultrasound images, subjective color Doppler ultrasound images, and the fine needle aspiration cytology of 325 nodules of 250 patients (age ≥ 18 years) were collected and analyzed. Hypo-echogenicity than adjacent strap muscle, micro-lobulated or irregular margins, micro- or mixed calcifications, and taller-than-wide shapes were considered as a suspicious malignant nodule in grayscale ultrasound. Marked vascularity was considered as a suspicious malignant nodule in color Doppler ultrasound. The Bethesda system for classification of thyroid nodules was used for cytopathology. Results: With respect to the results of fine-needle aspiration cytology for detecting suspicious malignant nodules, for grayscale ultrasound, subjective color Doppler ultrasound, and combined gray-scale with subjective color Doppler ultrasound, sensitivities were 0.564, 0.600 and 0.691, respectively and accuracies were 0.926, 0.919 and 0.959, respectively. Suspicious malignant nodules detectability for grayscale ultrasound, subjective color Doppler ultrasound, and combined gray-scale with subjective color Doppler ultrasound were 0.09-0.56 diagnostic confidence, 0.08-0.61 diagnostic confidence, and 0.063-0.7 diagnostic confidence, respectively. Conclusion: The combined gray-scale with subjective color Doppler ultrasound-guided fine-needle aspiration biopsies are recommended for the diagnosis of thyroid carcinoma. Level of Evidence: III.


Resumo Introdução: A citologia da punção aspirativa com agulha fina é preferida para nódulos tireoidianos no pré-operatório, mas apresenta desvantagens de resultados falso-negativos e falso-positivos. Objetivo: Comparar o desempenho diagnóstico da ultrassonografia em escala de cinza, do doppler colorido subjetivo e da combinação dos recursos da ultrassonografia em escala de cinza e do doppler colorido subjetivo na previsão do carcinoma da tireoide com os resultados da citologia da punção aspirativa com agulha fina como padrão de referência. Método: Dados de imagens de ultrassonografia em escala de cinza, imagens subjetivas da ultrassonografia com doppler colorido e citologia da punção aspirativa com agulha fina de 325 nódulos de 250 pacientes (idade ≥ 18 anos) foram coletados e analisados. A hipoecogenicidade da musculatura adjacente, as margens microlobuladas ou irregulares, as microcalcificações ou calcificações mistas e os formatos mais altos do que largos foram considerados como um nódulo maligno suspeito na ultrassonografia em escala de cinza. A vascularização acentuada foi considerada um nódulo maligno suspeito na ultrassonografia com doppler colorido. O sistema Bethesda para classificação de nódulos tireoidianos foi usado para a citopatologia. Resultados: Com relação aos resultados da citologia por punção aspirativa com agulha fina para detecção de nódulos malignos suspeitos, as sensibilidades foram de 0,564, 0,600 e 0,691 para a ultrassonografia em escala de cinza, ultrassonografia com doppler colorido subjetivo e escala de cinza combinada com ultrassonografia com doppler colorido subjetivo, respectivamente, e as acurácias foram 0,926, 0,919 e 0,959, respectivamente. A detectabilidade de nódulos suspeitos malignos para ultrassonografia em escala de cinza, ultrassonografia com doppler colorido subjetivo e escala de cinza combinada com ultrassonografia com doppler colorido subjetivo foram de 0,09-0,56, 0,08-0,61 e 0,063-0,7 de confiança diagnóstica, respectivamente. Conclusões: A ultrassonografia em escala de cinza combinada com o doppler colorido subjetivo e biópsias por punção aspirativa com agulha fina guiadas por ultrassonografia são recomendados para o diagnóstico de carcinoma da tireoide. ABSTRACT

Objective: We aimed to evaluate the clinical profile and radiological findings of children with nutcracker syndrome (NCS) and to assess the association between the parameters. Methods: A retrospective analysis of the clinical, laboratory and radiological parameters of children diagnosed with NCS between January, 2011 and October, 2017 was done. Results: Of a total of 29 patients [19 girls, 65.5%] with NCS, having a mean (SD) age of 10.8 years, 72.4% had BMI <-2SD. Approximately half of the patients (51.7%) were asymptomatic. Left flank pain was commonest (9/29; 31%) symptom, followed by macroscopic hematuria (4/29; 13.8%). Isolated proteinuria was seen in 9 children. There was no significant difference between the symptomatic and asymptomatic patients in terms of Doppler ultrasonography findings. All patients were followed up conservatively, 5 received enalapril therapy for moderate proteinuria. Conclusion: NCS should be considered in children, especially with low BMI, presenting with orthostatic proteinuria and hematuria, with or without left flank pain after ruling out the common causes.

ABSTRACT

Objective To preliminarily evaluate the application value of SpyGlass direct visualization system in the diagnosis and treatment of biliary stricture after liver transplantation. Methods Clinical data of 4 patients presenting with biliary stricture after liver transplantation who underwent SpyGlass direct visualization system examination were collected. The examination, treatment and prognosis of biliary stricture were analyzed. Results The examination results of color Doppler ultrasound, magnetic resonance cholangiopancreatography (MRCP) and endoscopic retrograde cholangiopancreatography (ERCP) in 4 patients suggested biliary anastomotic stricture with intrahepatic biliary dilatation, and 2 of them were complicated with intrahepatic biliary calculi. Repeated placement of biliary stent under ERCP yielded poor effect in 3 cases. SpyGlass direct visualization system examination hinted biliary anastomotic stricture in 4 patients, 3 cases of intrahepatic biliary dilatation, 3 cases of intrahepatic biliary calculi, 2 cases of purulent bile and 3 cases of floccules within the biliary tract, 1 case of congestion and edema of biliary tract wall and 2 cases of local epithelial necrosis and stiffness changes of intrahepatic biliary tract wall. The wire could not be inserted in 1 patient due to severe biliary anastomotic stricture. Four patients were treated with biliary stricture resection + biliary stone removal + biliary end-to-end anastomosis, biliary stricture resection + biliary-intestinal anastomosis, ERCP lithotomy + biliary metal stent implantation, and biliary metal stent implantation + percutaneous transhepatic bile duct lithotomy, respectively. Relevant symptoms were relieved without evident complications. All patients survived during the follow-up until the submission date. Conclusions Compared with traditional imaging examination, SpyGlass direct visualization system may more directly display the morphological characteristics of biliary tract wall and structural changes within biliary tract cavity, which is an effective examination tool for biliary stricture after liver transplantation. In addition, individualized treatment methods may be adopted for different biliary tract diseases, which is expected to improve clinical prognosis of patients.

ABSTRACT

Objective:To investigate the correlation between the dynamic changes of the superior mesenteric artery (SMA) blood flow after umbilical cord milking (UCM)detected and feeding tolerance in premature infants.Methods:This was a prospective study a total of 88 premature infants delivered in Shanghai Jiading Maternal and Child Health Care Hospital from June 2018 to February 2020 were randomly divided into UCM group(45 cases) and early cord clamping group (ECC group, 43 cases). SMA blood flow indexes 15 min before the first feeding (prefeed) and 60 min after the first feeding (post-feed) were detected using the GE Voluson 730 Ultrasound Machine, including the peak systolic velocity (PSV), time-averaged mean velocity (TAMV), pulsatility index (PI) and resistance index (RI). The gestational age, birth weight, the weight at discharge, SMA blood flow indexes, time to achieve enteral nutrition, length of stay and incidence of complications between the two groups were analyzed using t test, Chi- square test or Fisher′ s exact test. Results:(1)There were no significant differences in the gestational age, birth weight, the mother′s hemoglobin before deliver, and the incidence of complications between UCM group and ECC group (all P>0.05). (2) Except for RI, all SMA blood flow indexes were higher in UCM group than those of ECC group.However, only prefeed TAMV[(24.74±11.47) cm/s vs.(20.24±7.71) cm/s], post-feed TAMV[(42.11±15.74 )cm/s vs.(28.31±9.99) cm/s] and TAMV change before and after feeding [(13.89-20.86) cm/s vs.(5.31-10.82) cm/s]were detected to be significantly higher in UCM group than those of ECC group (all P<0.05). (3) Multivariable linear regression analysis showed that the gestational age, birth weight, PSV elevation and PI reduction were negatively correlated with the length of adequate feeding (all P<0.05). (4) The time to achieve enteral nutrition[(11.75±7.36) d vs.(13.38±8.12) d] and the incidence of feeding intolerance [13.3% (6/45 cases) vs.20.9%(9/43 cases)] were lower in UCM group than those of ECC group, although no significant differences were detected (all P>0.05). Conclusions:UCM increases the hemoglobin level in premature infants, which is beneficial to feeding tolerance of premature infants by improving SMA blood flow.
